What school will my child attend?

The Mantua Township School District is a K-6 district with Centre City and Sewell Schools housing grades K-3 and the J. Mason Tomlin School housing all 4th, 5th and 6th grade students.

In order to assign students in grades K-3 to the Centre City and Sewell schools so that there is efficient use of classroom space, the board of education has divided the township into two school sending areas. These sending areas are outlined below.

What are the registration procedures?

Parents and students who are new to the district are to follow the following steps to complete the registration process:

Step 1 Determine the school of attendance (see What school will my child attend?). If you have any questions, please contact the Superintendent’s Office.

Step 2 Visit the appropriate school to obtain the proper registration forms.

Step 3 Complete the registration forms and return them to the school office. These forms will include:

  • Pupil’s Personal Data
  • Health History Form
  • Enrollment Residency Questionnaire
  • Letter authorizing the release of all academic and health records

Step 4 In addition to the above Personal Data Form, the school office will require copies of the following:

  • Official birth certificate (a copy must be made at the school)
  • Immunizations
  • Recent proof of a physical examination (physical must have been completed within a year of the date of enrollment)
  • Proof of residency (property tax bill, lease, bill of sale, deed, settlement papers)
  • Pupil Transfer Card from previous school (should be mailed directly to school)
  • Custody papers, if applicable

What are the health requirements?

All students are required to have the following immunizations:

DTP (diphtheria, tetanus and pertussis) Polio

Mumps, Measles, Rubella (MMR) Hepatitis B series completed

Varicella (chicken pox) HIB vaccine


What should I know about No Child Left Behind (NCLB)?
